Customizing the GoPets collar (cats and dogs):

Here's the template:

The blue on the templates is the back half of the collar, the red is the front half of the collar, and the yellow is the tag hanging from the front of the collar.

This is how the template looks wrapped on a model:

So, to make a collar without a tag you make the yellow area transparent. To make a collar with a tag, gem, bell, etc etc. You just place that image in the center of the yellow rectangle.
